Who Must File

Applicable Large Employers (ALE), generally employers with 50 or more full-time employees (including full-time equivalent employees) in the previous year, must file one or more Forms 1094-C (including a Form 1094-C designated as the Authoritative Transmittal, whether or not filing multiple Forms 1094-C), and must file a Form 1095-C for each employee who was a full-time employee of the employer for any month of the calendar year. Generally, the employer is required to furnish a copy of the Form 1095-C (or a substitute form) to the employee.

Step 1: Start ez1095 Software

1.1 Please install the correct version.
For example: if you need to print Year forms, please install ez1095 .

1.2 After you installed the ez1095 software, you will see a new shortcut created on desktop. You can double click this shortcut to start ez1095 software.

If you are using the trial version of ez1095 software, you will see the key input pop up screen. You can enter the key code here or click the "TRY DEMO Version" to try this software for free. (Note: The trial version will print TRIAL image on forms.)

Step 2: Set up Company Information

- Start ez1095 software, click the top menu "Current Company" then the sub menu "Company Settings" to view company set up screen.

- Enter company information

- Click the SAVE button to save the change.

Step 3: Add Tax Form 1095-C

with ez1095 software

you can enter the form data manually by following the steps below;

or you can roll forward previous year's ez1095 data;

or you can import 1095-C data from spreadsheets;

or you can import ACA form data from the XML efile documents.

3.1 Click the top menu "Current Company" then the sub menu "Form 1095-C" to view 1095C screen.

3.2 Click the bottom link "Add 1095 C" to add a new form.

3.3 Follow IRS guide and fill in form data

3.3.1 Part I Employee: Enter employee data carefully.

3.3.2 Part II ALE Member Information: Follow IRS guide and fill in data.

3.3.3 Part II Covered Individuals: Complete Part III ONLY if the ALE Member offers employer-sponsored, self-insured health coverage, including an individual coverage HRA.

If your company does not provide self-insured health coverage, leave Part III of Form 1095-C blank. Do not select any options or enter any information in this section.

Step 4: Print Tax Form 1095-C

4.1 Select forms and click the PRINT button.

4.2 You can print optional shipping address to mail forms with #10 double windows envelopes easily.

4.3 You can view/edit printer options if needed

Step 5: Print Tax Form 1094-C

Click the top menu "Current Company" then the sub menu "Form 1094-C" to view 1094-C screen.
