Open ezW2, in "Current Company" tab, select "efile W2" -> Generate State Efile for Ohio.

Please use box 21, 22 , 23. Put four digit number in box 23, the wages in box 21 and tax withhold in box 22. See the image.

Please use box 18, 19, 20, 23. Put "City" or "County" or "Other" in box 23, the wages in box 18 and tax withhold in box 19, name in box 20. See the image.
